The ghost icon. does nothing, can't get rid of. ??

Hi,

I have a custom icon that does nothing, only appears when I add icons to the
menu bar and that vanishes if I go into customize mode (so I can not drag it
from the menu bar! I want to get rid of it but don't know how. It must be
grouped with one of the icons I have and inadvertently placed on the menu
bar when I place another icon there. or so I figure. Is there somewhere I
can go to see custom icons (tools) and get rid of this do nothing icon?

How it got there:

I had set up some macros and icons a while back and some macros got removed
or changed an some icons lost the macros associated with them.

Any help will be greatly appreciated!
