I tried folding the 11x17 paper in half first and then printing each page one at
a time, but after printing the first page, the printer creased the fold very
badly.

Have a look at http://www.mvps.org/word/FAQs/Formatting/BookletPrinting.htm
It won't do what you want however. The only real solution is to use the
relatively expensive print utility ClickBook from www.bluesquirrel.com, but
you need to be printing a lot of booklets to make this a viable option.

The position with Outlook is even worse. Lotus Organizer from the earliest
version could print to the most complex layouts - for loading into a paper
filofax - Outlook has never been able to do this. Clickbook does the
business, but I object to its cost - they have just asked me to buy a $10
upgrade to add footers!
