The pagination for the document is set up to use that of Heading 2 (i.e.
1.01-1, etc.) and this is working fine.
There is no body text for heading 1 Chapter titles - the chapter titles
merely introduce the subsequent section titles, which is why the pagination
is set up for heading 2. And therein lies the problem. When the Table of
contents is created, except for the first chapter title which correctly
displays 1.01-1 for the page number, subsequent chapter titles display the
page number for the previous section (i.e. 1.03-1 instead of 1.01-1).
I assume that since the documents page breaks are set up for the section
titles, the TOC interprets these as the beginning of a new chapter. How do
I work around this and get the TOC to display the appropriate page number.

Best and safest solution is to make your Heading 1 style 'Page Break
Before'.
I'm not clear in your case whether in fact your page breaks are section
breaks that you can't change. If so, you might get away with it if you
rebuild the contents from scratch (there seem to be some in-built kludges
that will sometimes recognise the style doesn't start till the following
page, but if you insert the break after the hidden bookmarks for the TOC
are in place they'll get picked up anyway.)

The problem is the page number displayed for Chapter 2 (and subsequent
chapter headings). It should be 2.01-1 instead of 1.03-1 . The page
numbering in the document is handled by odd page section breaks. In the
document the page numbers are displayed in the headers (odd and even). The
page numbering is based on header 2 rather than header 1. I assume that is
what is causing the problem with the Table of Contents.
A small sample of the document is below. The page number for the document
is correct (2.01-1).

The case management system is designed to handle and control documentation
which comes before the Commissioners in an official capacity. Matters
For the moment I have resorted to not displaying the page number for header
1 in the Table of Contents. Any suggestion on how to overcome this would be
appreciated.
