You don't say if you are using numbered headings or not, but I have had the
same problem when using numbered headings.
The fix (probably a work-around) was to:
1. Go into Format|Style for each of the headings that would appear in the
TOC,
2. Press the Format button and select Numbering from the drop-down.
3. On the "Bullets and Numbering" window, select the Outline Numbered tab
and click on the Customize button.
4. On the "Customize Outline Numbered" window, click on the More button and
select "Space" in the "Follow Number With" combo box.
This replaces the tab that Word puts between the number and the text of the
heading with a space. It *also* stops Word inserting the extra tab in the
TOC.
This may not be the kosher solution - and hey, it may not even answer your
particular problem - but it worked for me.

This problem is known as the "Jason Tab" for reasons that are hidden in the
mists of time. Take a look at the following article for a full explanation.
Whenever I update my Table of Contents it acquires unwanted tabs, and I have
to press Ctrl+Q to get rid of them

Just check that in Insert>Index and Tables>Table of Contents you have the
Format box set to "From Template". Any other setting uses Word's built-in
hard-coded defaults, which will add unwanted tabs to your TOC style
definitions.
