There should be _two_ files on the computer:

C:\Program Files\Microsoft Office\Office12\Document
Parts\1033\Building Blocks.dotx

and

%appdata%\Microsoft\Document Building Blocks\1033\Building Blocks.dotx
(the "environment variable" %appdata% represents the hidden folder
C:\Documents and Settings\<your name>\Application Data in Windows XP,
or C:\Users\<your name>\AppData in Windows Vista). You may need to go
into Windows Explorer's Tools > Folder Options > View dialog and
choose to show hidden folders to be able to find the %appdata% folder.
The one under the Program Files folder is the original from the
installer, and that's the one that would be replaced by a repair or
reinstall operation. The one under the %appdata% folder is the one
that Word actually loads, and which is apparently damaged; that one is
never affected by repair or reinstall because it has a later date than
the installer's copy.

Shut down Word, delete the copy in the %appdata% folder, and restart
Word. That should make a new copy of the one from under Program Files,
and the galleries should work again.
