Have created a form template with blank line for filling of text. The line
will break when text are inserted. Please kindly advise what should be the
right way to create a blank line for text insertion without the line pulling
apart?

If you are filling the form on a PC why do you need the line? However, put
the form field in a table cell of fixed width and height and set the bottom
border of the cell to emulate your line.
