How do I create a survey in Word?

I am trying to create a survey to gather information for a business that I
would like to open So that I can determine if there is a need for the
business or not. But, I would like to get a little feedback before I go
ahead with my plans.

I am at a loss for where to go and what to do. I know what I want in the
survey just not how to go about it. I would like to be able to track the
information. I will have to do my survey on paper, because I will need the
participants to be local to my area. I will have to do that in a different
way. Any help would be greatly appreciated.

I am using Word 2003.

Hello Lost9

You need what Word will call a form, but if you're going to fill it in using
pen and paper, it does not need to be very sophisticated in Word's terms.

Start here:
I'm designing a form that needs to have "fill-in-the-blanks" lines. What is

By the way, if you later decide you want an on-line form to run from a web
page, there are any number of businesses that offer free or cheap ways to
conduct surveys on-line. Try searching in Google for free online survey and
you'll find heaps of them.
