Automatic Duplication when filling out a form

I have created one form, which needs to be distuributed amoung a group of
people to fill in the blanks and check appropriate boxes. When I tried to do
a test run by filling in the blanks on a one form then save it; then type
over the same form with different information, I receive a duplicate of the
same information from the first form. How do I stop this from happening?

Instead of reusing the form like this, create a *template* with the required
form elements in it.

Create a template as recommended by Stefan. Create a new blank form from it.
