Select Columns from the Format menu and setup the number and dimensions of
the columns to suit.

Although it would be possible to create a sheet like that with Word's
table feature, it would be much less work to do it in Excel --
especially if you're planning to make entries on the computer, rather
than printing the sheet and filling it by hand.
