How do I create a Word Index to include all words in a doc?

I'm new at creating Indexes and I need to create one to include ALL words in
a 31 page document. Not sure how do go about it. Seems like the text has to
be marked in the document, but I can't seem to select the WHOLE document to
do this.

Note, however, that an index containing all the words of a document is
unlikely to be useful.
