If using the standard Heading styles (recommended and the default setting)
then you need to make sure your headings are indeed in those styles.
If you are using custom styles or have used TC fields, then you need to use
the Options button in the TOC dialog to either assign styles to TOC levels
or indicate you are using Table entry fields

The doc is approximately 38 pages long. The text contains headings up to
four levels deep (ie Heading 4). They have already been formatted. I have
created a table of figures at the end of the document listing four figures
within the document. I created the table of figures to list the figures as
hyperlinks to the figures (no probs creating that). At the beginning of the
document (behind the header page), I want to create the TOC. When I go to
Insert...Reference...Index and Tables...'Table of Contents' tab...'OK'
button, I get the following error: Error! No table of contents entries found.
That is my dilemma
Note that to get Heading 4 to appear you'll need to edit the default TOC to
include one more level - it would be worth using Options to check you do
actually have Heading styles mapped to TOC levels.
Check also that you have applied Heading styles as paragraph styles (select
whole paragraph and apply style - don't swipe part of the para. If using
built-in levels these come from the paragraph style, not the pseudo
character style that can be applied to part of a para.
