this is a fresh install of windows vista ultimate w/ sp1, this is the second
reinstall, I tried the update right after the install finished to make sure
it wasn't any of the drivers I installed before.

Did you format & reinstall Vista (SP1)?
What anti-virus application or security suite is installed and is your
subscription current? What anti-spyware applications (other than Defender)?
What third-party firewall (if any)?
Has the machine been connected to the internet or any networks and/or have
you inserted a USB key without an anti-virus application being installed
and/or without a firewall enabled?
