Windows updates won't install, and icon issues seem to be related

I have found other postings on various websites that have described this
exact problem I'm having, so I know this isn't a complete one of a kind
fluke, but have not found any solutions anywhere.

Almost all the icons on my system are showing the default icon for an
"unknown file type". This is happening for all Office documents, PDF files,
and also shortcuts to these files. Some file types have retained their
correct icon, but not many. For applications that are affected, it is
affecting both shortcuts to documents and to the application itself - for
example, all MS Word documents on the desktop as well as the shortcut to the
program in the "start" menu. However, the original program file at C:\Program
Files\Microsoft Office\Office12\winword.exe shows the correct icon.

All of the icons still work fine - both a document and a shortcut to a
document will call up the program and open the appropriate file in the
application. If that were the only problem, I could live with it ...

However, at the same time that this happened, I'm finding that most Windows
Upates won't install (and other programs as well - like an update to the
Adobe PDF reader). All the updates that will install have been installed.
The rest say "downloaded, ready to install" on the Windows Update page but
when I try to install them it says "installation failed" at the end. Most
Windows Updates just fail to install, but some - like Silverlight - throw a
message during installation saying that it needs to find the .msi installer
file.

In the case of Silverlight, if I try to install from the WindowsUpdate
website, it asks for the location of the Silverlight.msi installer before it
can continue. I've searched my entire system and it doesn't exist. However,
if I go to "add/remove programs" and try to uninstall the current version, it
won't uninstall - I get the same message. Lastly, I tried going to the
Silverlight website and downloaded the new .exe installer to run. Still no
luck.

For Adobe, I get a message that an update to the Adobe Reader 8 is
available. It downloads, but then I get an error during install and it
aborts. I get a reminder every few days about the update but it never
installs.

I have tried a system restore to a point in time before this problem first
happened, with no success. I ran a "checkdisk" and told it to fix issues,
and I've tried running a "registry cleaner" program. No luck with anything
I've done.

Any suggestions would be greatly appreciated!

The very first thing you should do is to find out if there is any
malware resident. And, running a 'registry cleaner' can cause more
issues than it resolves.

Not sure by your post if this is XP or Vista. Besides the specific
edition of Windows installed, what Service Packs have been applied so far ?

In the meantime, see if you can download MBAM, install, update it, and
then run a Quick scan: http://www.malwarebytes.org/mbam.php
Click the ' Download free version ' button.
Once it's downloaded you may need to change it's name to *anything*
other than mbam-setup to get it installed.
Once it installs allow it to attempt to update. If it can not, then do a
Quick scan anyway. Allow it to remove what it detects.

*IF* it can not install and the system was able to update last month,
then try running the Microsoft Malicious Software Removal tool IF you've
previously allowed Windows Update to install it.
In Vista, click the Start orb, type in mrt in the Start Search field.
Then *right* click mrt.exe and choose Run as administrator.
In XP, click Start > Run > enter mrt in the Open field and then click OK
or press Enter.
When MRT opens click Next, then put a mark next to Full scan.
Click Next again. The scan will take awhile to complete but *should*
allow the installation of MBAM and/or allow you to have the system
scanned by an online scanner.

If the MRT has never been installed nor updated, then I doubt you'll be
able to download and install it as the latest 'versions' of malware are
acutely aware of the MRT.

Please state your full Windows version (e.g., WinXP SP3; Vista x64 SP2) when
posting to this newsgroup. In this case, also state your IE version.

What anti-virus application or security suite is installed and is your
subscription current? What anti-spyware applications (other than Defender)?
What third-party firewall (if any)?

Has a(another) Norton or McAfee application ever been installed on this
machine (e.g., a free-trial version that came preinstalled when you bought
it)?

Sorry, I thought I had mentioned that. System has XP SP3 with all updates
(except for the five or six latest security updates that won't install). I
have the full Windows Live OneCare (fully updated) running, and have done two
different malware scans, all negative.

I was running IE7 when the problem started, but I've upgraded to IE8 ...
which installed and is working fine even with this problem!

One more thing ... a few different websites I've found said to either set the
icon size to one point larger or small, and the reset them, or set the
desktop color depth to 16 bit and back to 32 bit, which will rebuild the
iconcache.db file. I tried both and neither solution had any effect.

Can you access http://windowsupdate.microsoft.com?

You may have made things worse doing that. If you think your Registry needs
to be "cleaned" or "repaired," read http://aumha.net/viewtopic.php?t=28099
and draw your own conclusions.

Hi PA Bear - thanks for helping me with this.

I had used McAfee for a while but I switched to OneCare over a year ago (and
yes, the subscription is current).

I just finished running a scan with Malwarebyte (suggested by MowGreen). The
program installed and ran fine but didn't find any issues.

I've used Ccleaner on other machines and it has miraculously solved other
problems with no ill effect. I don't think it has made anything worse here,
but it hasn't solved my problems either.

I can access the WindowsUpdate site just fine. As I said below, some
updates do install and have installed. The others download but won't
install. When I go to the page with the updates, they are all listed as
"downloaded, ready to install"

Just FYI - I am ex-Microsoft. I worked in the Windows Media group for nine
years, in program management, test, and business development (I mostly worked
on WMA and WMV codecs).

1. Right-click on the following link and download the McAfee removal tool,
saving it to your desktop:
http://download.mcafee.com/products/licensed/cust_support_patches/MCPR.exe

2. Close all open applications (i.e., anything with an icon on the task
bar).

3. Double-click on the file you saved in #1 above to run the utility. Don't
TOUCH your keyboard until the run completes, then reboot.

4. Test.

If still no joy, examine your WindowsUpdate.log (or check the Update History
section at http://windowsupdate.microsoft.com) for errors associated with
the failed installs. Post any errors found in a reply to this thread.

The McAfee removal tool installed and ran fine, but no change to my system.

There are five updates on WindowsUpdate that won't install. The log entry

Again, I don't think it is clear *which* package the message is referring to,
so again, I would use ProcMon to help clarify that.

OTOH if all you want to do is *do* the updates that AU has identified for you
as necessary to be installed, a more effective approach IMO would be just
taking note of them and downloading and installing them manually.
Particularly in the case of the .NET updates in my experience when they go
wrong much necessary diagnostic information is hidden from the user that
a manual install makes more apparent, if it is needed.

Otherwise you can still help yourself out at this point by searching for all
files that the update created or changed. E.g. search for all files on the
harddrive which have a timestamp within the hour that the update was run,
sort them by the timestamp (easier when your Date is YMD and your Time
is 24:00, e.g. when your search tool is dir/a/s) and find some of the diagnostics
from each install attempt which the update process is hiding from you.

I had a similar problem with MS silverlight. I uninstalled and
re-installed office which corrected the problems that I was having with
Adobe, Visio, and other updates. However, I'm still unable to get beyond
the installtion issues of the Silverlight Update.

It turns out that my problem is really an OS issue - something got corrupted,
and I'm not able to UNINSTALL any programs. Everything else is a side-effect
of this.

With the Windows Updates, some updates will install, but any of the updates
that require older components to be uninstalled first will fail during the
install process. I cannot uninstall or modify any programs by using the
Add/Remove programs in Control Panel. (However, new programs that were
installed AFTER this system corruption happened will uninstall fine through
add/remove programs.)

If I use the “Windows Install Cleanup Tool” to force a program to uninstall,
and then reinstall it from scratch, then the program seems to work fine after
that (this worked for Silverlight and the Adobe reader). This is a workable
solution for small applications like these, but I’m hesitant to use the
cleanup tool to uninstall complex programs like Microsoft Office, Visual
Studios, Quicken, Microsoft Expression – all of which are affected by this
problem - as updates, service packs, and security patches won’t install on
these programs (because the updates need to uninstall something else first,
which is where it fails). Somehow the icon issue is just a side-effect of
all this. I was able to prove that with the Adobe Reader ... reinstalling the
program fixes the icon problem for that program.

When the issue first happened, I tried a system restore to an earlier point
in time but that didn’t fix the problem. I also ran a checkdisk – which did
report some cross-linked files and some bad sectors which were re-mapped, but
that also did not fix the problem. I ran a registry cleaner and I tried the
“repair” features in a few other programs like TweakUI.

I have run four different mal-ware scanners, none of which have reported any
issues at all with my system – I am fairly certain that the problem is NOT
due to a virus or malware. I do have an open case with Microsoft Technical
support, they have been very helpful (but so far they have not found a way to
fix it ...)

Picathartes - Microsoft Customer Service has opened a service case for me and
they have been very helpful, but they have not been able to solve the issue.
The consensus is that I had a disk error that caused a corruption. They have
recommended that I use the "repair" option on the original system disk - but
that would mean that I'd have to reinstall all service packs and probably
most applications. I intend to upgrade to Windows 7 when it is released, I'm
just going to wait since I'll need to reinstall applications at that time
anyway.

If MS do come up with a solution then please post it, I'd greatly appreciate
it.

We had a peaceful spell in the office this morning so I did a Windows repair
on my PC.
Repair completed successfully but all the issues remain.
Windows repair does not solve the problem :(

Wow, I am surprised - I was sure that a system repair would have fixed it.
Hmm.

Just a question - have you "cloned" your system disk? I did that a while
ago, to upgrade to a larger drive, and I read on another board that some of
the cloning software products will not copy the "uninstall" directory. I
don't know if this is true or not, but it's the only other lead I've found.
