Adaptec 2940 UW Boot Problem after adding 2nd UW disk

I have been running NT4 / SP3 WS on a 2940uw adapter with one wide scsi
(boot) drive successfully. After adding second wide scsi drive, at the
'blue screen' portion of the boot up I get an 'inaccessable boot device'
failure.

SCSI channel is properly terminated.
Booting in DOS and partitioning/formating/using the new disk works
fine.
SCSI IDs are 0 for boot (old) disk and 1 for second (new) disk.

Technet has an article that suggests using an active terminator to solve a
similar problem.

The BIOS version on the 2940UW should be 1.23
