Check to make sure the entire registry key at

HKLM\Software\Microsoft\Update Services is removed, since you've fully
uninstalled all components of WSUS 3.0.


Most likely this is specifically related to the registry values
"SQLServerName", et.al. in the key at
HKLM\Software\Microsoft\Update Services\Server\Setup

but it's just easier to purge the whole master key and ensure you're
starting with a clean slate.

I had already done that and was still getting the error message when I ran
the WSUS installer - "An instance with the same name is already installed on
this computer. To proceed with SQL Server Setup, provide a unique instance
name."

I ended up doing a system state restore from my backup tape in Directory
Services Repair mode (since this is a DC) and was able to run the WSUS
installer which allowed me to remove WSUS (including the database) then
reinstall it. Thanks, everything is working now.
