The only account that appears in both SBS server management and in active
directory is the administrator account.
As far as I know, all SBS users and groups have only ever been set up via
the SBS Server Management console
Am I missing something or better still, is there a way to get AD to sync
properly with SBS Server Management ?

If the SBS wizards were used to set up the user accounts, they should be in
this OU: (not the 'Users' OU)
Server Management | ADUC | <yourdomain.local> | My Business | Users |
SBSUsers

If I open 'server management' and go to the users section, they are all
listed as I'd expect.
If I go to start / administrator tools / Active Directory users and
computers, then expand the users, all i see is a bunch of weird secutity
groups , a few disabled users (like guest) plus the administrator account.
When i checked this on another SBS 2003 server at a different site, the
active directory users and computers console showed no users immediately
under users, but there was a subfolder called sbs (or sbs2003) and all the
SBS users were in there, but no weird ones at all.
The reason for wanting to find the users in AD is because I wanted to set
up the Send As permissions, although since then I have discovered the 'Send
on behalf of'' located via exchange general tab/delivery option button - and
this seems to have done the trick.
But I'd still like to know if the AD and SBS user lists should be the same.

I needed to use the main ADUC console in order to set the 'advanced' view
and thence have the secutity tab on properies so i could set the send as.
Finally fixed, but what a dogs breakfast these screens have become.

Organizational units are Active Directory containers into which you can
place users, groups, computers, and other organizational units. Without
OUs, all users would be maintained and displayed in a single list, the Users
container, regardless of a user's department, location, or role.
When you look at ADUC, all the 'sub-directories' under it are the
Organizational units. During the install of SBS, a new OU is created called
MyBusiness. This (and its sub-directories) is where all users, computers,
distribution groups and security groups are placed when you run the SBS
wizards. These OUs allow Group Policies to be easily assigned to them.
If SBS has been installed and maintained properly, you should not see any
users under the 'Users' OU in Active Driectory. All user will be under:
Server Management | ADUC | <yourdomain.local> | My Business | Users |

I normally do everything via the Server management interface and use the
wizards, so as you say everything should be set up correctly
