The user uploaded a document to a document library from a shared network
drive, when the "New" document appeared on the site, the same user opened
the document and it appeared as a completely different document. I logged
onto the site from a different machine and opened the "New" document and
found it was the correct document.
Does anyone know the cause of this issue?

Old document was present on the site with the same name.
New document added, overrides the old document but the old document is still
in the cache.

Of course if the reason I gave *was* the reason, you are likely to have
another user complaining that his document has been replaced with something
else.
Unless you have versioning on that document library, the first person's
document is gone for good (hopefully he has a copy on his hard disk still).
Avoid this (perhaps) in future by turning off the normal default "overwrite
if same file name".
