Sending Outlook form from Internet

I'm kind of a newbie to messaging, and I'd like to do the following:

On an Internet page, I'm going to collect data from the web page (via a
user). Then, when the user clicks on a "Request" button (or something),
I'd like to populate a custom outlook form and send the form to the
librarian(receiver). The user clicking on the "Request" button wouldn't
see the Outlook form. All the info would just automatically be send via
the custom Outlook form. Is this scenario possible? any help on this
would be greatly appreciated!

Clicking on Request could fire a VB action on your Web server. The VB
action can populate an Outlook form and send it. A better place to get
info on the Web side would be the ActiveX, VBScript, IIS or even FrontPage
forums. You could also use VBScript on your Web page. There are examples
on how to use VB and VBScript with Outlook at
