You can remove the LDAP address book by starting Outlook and selecting Tools
> Email Accounts > View/Change existing directories and address books.
Highlight the LDAP address book and select remove.

If you're using Outlook 2003, choose Tools | E-mail Accounts and either remove or change the settings for the LDAP address book that is apparently part of your profile.
