Make a Custom Sticky Note

Hi,

I want to make a custom sticky note form so that when I bring up a new one,
it already has certain fields in it.

If I can't use fiekds, then just plain text will do.

When I go into Design Forms, though, sticky note is not an available option
in Standard Forms.

When I scroll down to the the Notes folder and click on ipm.sticknote in
there, I get an error message saying "that form cannot be displayed"

Can anyone help?
