Outlook Email to send data to SQL Server

I am feeling very dense today so need to ask this question here where
someone whose brain is still working may be able to help me out.

Is it possible to send an email to a user in Outlook and have a form
on there where they will enter comments etc and then save the
responses on a SQL Server database. I can't think how to do this. I
would like to do it in .NET, is there an automated way in Outlook
forms to do this or do I need to program the form myself and have the
sender button do the database stuff myself.

As I said feeling pretty stupid today, it is Monday so any pointers
would be nice :)
