Custom Form - Multiple Names & Phone Numbers

I would like a form that I can have 1 contact which represents a company
location (like a store) but be able to enter multiple employee name and
extension information displaied in a table/list view

This can be done by adding the Outlook View Control to a form and adding code to set its Restriction property. See http://www.outlookcode.com/d/ovc.htm

FYI, there is a newsgroup specifically for Outlook forms issues "down the hall" at microsoft.public.outlook.program_forms or, via web interface, at http://www.microsoft.com/office/community/en-us/default.mspx?dg=microsoft.public.outlook.program_forms
