Fill-in-the-blank messages from Outlook

Good afternoon all ...

I have a question ... is it possible to create a fill-in-the-blank type
message for outlook? For example:

A vacation request

A schedule change

can someone point me in the right direction?

What version of Outlook?
You could certainly do this with templates but outlook 2007 has InfoPath
integration that makes it much easier.

Probably custom Outlook forms with Outlook 2002.
You might even find a sample form that suits.

Note that custom message forms are usually practical only in an Exchange environment where form designers have permission to publish to the Organizational Forms library.
