Creating a form using Outlook.

I'm trying to create a form that our company can use. The form will be used
as a communication file for 15+ part time employees. For instance: the
morning crew is given an important message. That message needs to be given
to all employees for the day. I would like to create a template that will
allow me to do that AND have them type their signatures next to their names
to imply that they have received the information.
