There is no installation procedure. It's built into Outlook. Tools | Forms | Design a Form.

(Outlook 2003): But, the system says it is not installed. "Unable to open
survey form because some files are missing on your machine. The Microsoft
forms designer tool must be installed."

"survey form"? Do you get the same message if you try to open one of the built-in forms in design mode? Is this copy of Outlook running on Terminal Server? Try running Outlook setup to see what components are installed.
