Outlook 2003 with small business manager

I am trying to customize my contact form by utilizing the
edit function in the forms options menue. I am having
difficulty saving the edited form as the program tells me
that I must have a name in it to save it. I want to save
it as a default template for that part of of Outlook
Business Contact Manager. I also want to be able to
import data from Excell into this form including the
custom fields contained in the edited contact form. I
cannot seem to get that work either. HELP!!
