Creating online questionnaire

Does office (or Microsoft for that matter) have software that can be used to
design an online questionnaire? I am not proficient at writing code. Some of
the features I would like in the questionnaire are:
1) Conditional branching questions
2) Incorporate various response types (bullets , dropdown choices, text boxes)
3) Hyperlinking
4) Able to take the reponses and import (collate) them into a "summary
document", preferably resembling a MS Word document.

I have looked at Adobe's Captivate 4, but it does not have all the features
I needed.

I've just started using LimeSurvey for this kind of thing (limesurvey.org).
I wouldn't expect a wordprocessor to be able to do this.

I like Surveymonkey.com. you can create a link to you survey from an e-mail
