How importing Access file to OUTLOOK contact

How can I import Access database tables into an OUTLOOK contact list, where
that contact list is not the standard form?
When I use import/export from an MS Access file, I always get the standard
contact form, and there is no way to change the form to one of my own (that
I created using design form).

Have you change your default form for the folder? Try changing the message
class of your new form using the instructions here:
