Automation Error [-2147008507][80074005]

We are
- Running VB Client, VB MTS Services, Oracle
- Reading and disconnecting a recordset on the server
- The recordset return 0 rows, we insert a record into it on the server
do some initialization and return it to the client.
- This works except for on some rare occasions where we get
Automation Error [-2147008507][80074005].when we are returning the
recordset to the client.
- Once this happens, we have to restart MTS to get it to stop
happening.
