Right click the web site, select Properties then click on the "HTTP Headers"
tab. On the bottom of this property page, click the "MIME Types." button.
A list of the custom MIME types set on this computer will now be displayed.
On the right side of the MIME type window click the "New." button. Type the
extension ".OFM" (without the quotes) into the first text box. In the "MIME
type" text box, type "text/html" (without the quotes) and press the "OK"
button on both of the dialogs, and "Apply" on the Properties window.

You can verify in the IIS web logs the sort of 404 response you are getting
on requests to .OFM files
If it's a file that needs to be translated to display, then you need to:
- look up the .OFM ScriptMap to find the ISAPI DLL to enable
- create a Web Service Extension and enable the ISAPI
If it's a file that needs to be downloadable
- create a MIMEMap entry for the extension that needs to be downloaded
