Signed/Secured ActiveX does not work on Default security settings

Trying to find best place to ask question on ActiveX that we created and
signed with a digital certificate but won't download or install when the
ActiveX setting in IE6 "download signed ActiveX controls" is set to
"Prompt".. nothing prompts and the ActiveX won't download and install!
However if you set this option to "Enable" it works flawlessly.

Can someone please help? Also I have signed another ActiveX control that
works just fine with this set to prompt.
