Sharepoint 2003 User Issue: User is ex-employee who has returned, cannot add her back to sites

We are using Sharepoint 2003 Portal Server. I am having an issue
adding a user back to sites. She left the company awhile back and
therefore her permissions were removed from various sites. She is not
listed on any of these sites, but when I try to add her back I get the
error message "Can not add the user because a user with that name
already exists. " What do I need to do to add her back again?
Someone suggested I may have to remove her old entries on the database
level - if that is the case, how would I go about doing that?
