OWA missing Icons

I able to login to OWA but i'm missing all the icons. I have the
famous red X were all they button should be. Anybody experience this?

I have run into this same problem, kinda of. I had to reinstall my Exchange
Server 2003, I had my active directory domain controler die. When I
reinstalled the domain controller, I had to rejoin the Exchange 2k3 server
to the domain. I had to manually uninstall exchange 2k3, and finally got it
reinstalled. This is on my test network so there is no backups running. I
don't want to wipe the Windows Server 2003 computer because of limited
number of installs for this Version of Windows.

Today I installed Service Pack 1 for Exchange Server 2k3, thought it might
help but still no luck.

When I access the web based Outlook on my Exchange server I get little red
X's on all of the pictures and none of the links seem to work.

When I access the site from with in the network using the computers netbios
name or right from the server everything looks normal. What could be
causing this error/problem?

Whenever you run into problems with the icons not coming up, or are getting
prompted over and over for credentials(as one icon at a time comes up), it
is always something wrong with the Exchweb directory. The Exchweb
directory is where all of the icons and controllers are located. By
default, you want the Exchweb virtual directory, in IIS, to have only
Anonymous authentication. If this is not the case, then change it to this.
Don't forget to recycle the World Wide Web Publishing Service. If this
does not work, then remove Anonymous, and put Basic(w/ same domain name as
Exchange & public have). Again, don't forget to recycle the World Wide Web
Publishing Service. If this does not work, then check the IIS
logs(c:\winnt\system32\logfiles\w3svc1\). Check to see what errors you are
getting(401, 404, 500, etc...) Also, check to see if either URLScan or
IISLockdown are installed.

The authentication settings for OWA should be the following:

Exchange & Public(through Exchange System Manager): Basic(w/ Default Domain
= <internal netbios name of AD domain>)
Exchweb & all subdirectories(through IIS): Anonymous (or Basic w/ Default
Domain = <internal netbios name of AD domain>)
