Command.ext: Exchange Custom Command Extensions Sample -- Where is it?

I am looking for the Command.ext sample code referenced in MSDN

http://msdn.microsoft.com/library/default.asp?url=/library/en-us/exchcli/htm
l/_mapi1book_sample_command_extension.asp

I have installed all the sample code from both MSDN Universal and the
Platform SDK. I have also looked at ftp.microsoft.com/developr with no
success.

Please help, I am tearing what is left of my hair out over this!

The MAPI samples were missing from some versions of the platform SDK; go
to download.microsoft.com, product='platform sdk', and you can find them
that way.
