ERTN does not send emails when there are emails to receive

We are running Exchange 5.5 sp4 and using ETRN to send and
recieve emails via dial-up internet access. We have it
set up to dial out every 30 minutes. There seems to be a
pattern where if there is messages to receive, we seem
unable to sent mail messages out. Anybody have any
suggestions?
