Help! Outlook form disappears when trying to install!

I am having a weird problem installing a custom form where the form seems to
be loaded for just a second, and then it disappears.

I am trying to install a custom form and link it to a public folder
containing contact items with custom fields. I am using Outlook 2000 and
Exchange 5.5 SP 2.

From Outlook, I right-click on the folder to show the properties page, then
click on the Forms tab and the Manage button. I then click 'Install', point
at my .fdm file, and open it. The Form Properties dialog is displayed,
showing the name, no categories or subcategories, and the correct message
class. I click OK, the form name 'Customers' appears in the right window
for about a second, and then it disappears!

I have owner permissions on the folder, and everything else looks OK. Any
clues as to what might be causing this?

Here's the latest:

I'm still having the problem, and it must be something with the Exchange
server settings. I've connected the same Outlook client to a different
Exchange server, and the same action with the same form works as expected on
the second Exchange server. I try the same thing again on the original
server, and I have the same problem.

To summarize: I am trying to install a custom Outlook form (.fdm) into a
public folder containing 1600 contacts with the same (custom) message class.
The form is found, but it is not installed into Exchange properly. The form
name is actually displayed in the 'Manage Forms' message window for a brief
instant (about half a second) and then it disappers.

I have full administrator permissions on everything (I think), but it still
doesn't work.
