does the person who's sending also have "send as" permissions? if they do,
that takes precedence...

I have looked for my permission's i.e. "a"  - I have admin rights although I don't use the admin user name.  I have removed the Send As from Administrators from the Mailbox Rights in AD.  When I tested this it still came up with person B name and not my:
A on behalf of B
 
I am i not looking somewhere!

There are two different settings. "Send As" and "Send on Behalf of". You want the latter.
With your testing, don't logon as yoursefl as you have admin rights so will always be able to Send As.
Set up a test account with no rights, then add permissions only as required until it works.
