I have had that begin happening for all accounts attempting to get in
via OWA, with no changes done on Exchange or IIS. The fix was to
change the anonymous user account on IIS for the Exchange virtual
directory and make sure it used 'password synch'. There was nothing
wrong with the old NT user, IIS just needed to fix something in its
metabase apparently.

We had previously see CDO begin failing on GetDefaultFolder where the
solution was to change the anonymous user account and make sure that
IIS DIDN'T use password synch!
