User Name length limitations

I have a user with 18 letters in her name. When I created the mailbox
she was able to log into it. She could send mail fine, but she was
unable to receive anything. When I created a shorter user name, and
the alias was shorter, it worked as expected. What is the limitation
for username/alias length?

The "alias" can be up to 64 characters in length.

Have you considered that the problem might be in the originating
system and not in Exchange?
