AIUI the 2003 spamfilter simply lets you use DNSBLs - you might want to
look at a relay in front of your exchange box, given the cost of
hardware/software and cals it seems an expensive option if your prime
reason is spam filtering.

We still work with exchange 5.5, but with SpamPal installed: freeware
utility to filter spam
It works very straightforward.

SpamPal is a POP3/IMAP proxy (http://www.spampal.org.uk/)
I prefered to install SpamPal on a different computer ("MailProxy")
Exchange Server checks POP3 mailboxes at MailProxy on port 1101
Because SpamPal is installed here, it listens to port 1101 and routes every
request to ISP mailbox on port 110
Mailbox is downloaded, checked for spam and routed back to exchange server
again.

Another benefit: installed NOD32 (anti-virus) on my MailProxy and configured
IMON (mail antivirus) to listen on port 110

So routing of mail:
-external client sending mail to our domain
-POP3 mailbox of domain at ISP
-MailProxy: spam is filtered and virus checked
-Exchange server
-internal user mailbox

works nice and filters +/- 80% of spam. With NOD32 installed, all mail is
checked at MailProxy: no need of client antivirus software (although also
installed there...)
Spammail can be finetuned with bayesian filters etc... but satisfying right
now

I have been running Exchange 2003 for a month or two and
I didn't see much improvement until I started running
Office 2003 with Exchange 2003. Then Outlook and
Exchange working together catch most of my SPAM.

The best part is, it works unlike the 20 or so SPAM
blocking software packages I tried.

Spam filter in Exchange 2003 is good, but you still need a third party spam
solution...

If you haven't already, you should look into Reverse DNS lookup, RBL, and
Message Filtering configuration on your Exchange 2003 server. Combined with
Outlook 2003 features, this really helps reduce the spam messages received
by the server and the client.
