Hi all.

I've recently been diagnosed as a diabetic. "So what?" you correctly
say, but I'm trying to write a speadsheet where I can keep track of my
sugar levels, meds, food intake etc. What I want, in a perfect world,
is a button on the top of the worksheet that opens a form. I can enter
the info I want, and it gets put onto the next blank row when I press
OK.

I've googled for excel forms but so far I've not seen what I want.
I've seen how to add to the quick access bar, I've found stuff I don't
comprehend, but no tutorials of the type I need. So, can anyone help
please?

I have 2003 at work, 2007 at home and I want to put the sheet onto a
usb stick so I can keep it up to date. So I suppose it's got to be
2003 format.

Many thanks.

Mike.

Microsoft shares a lot of templates. I searched for diabetic and found this:

http://office.microsoft.com/en-us/templates/results.aspx?qu=diabetic&av=TPL000

If those aren't what you like, you could search google. I searched for
"diabetic excel" and got lots of hits.

Hi Mike
What you're asking is a lot of details, maybe someone else will supply you with
a template, but have a look at this website > Section Excel and check
UserForm.Maybe that could be modified to your needs.

I'm also diabetic and I just keep track of my blood sugar and Insulin intake.
