Hi

I had the same problem in Excel 2010.

You need to go to File, Options, Add-Ins

On the bottom of the right panel you will see a box
Manage 'Excel Addins' Go...
Click on the drop down box and select "Disabled Items"
Click on Go
You should then see a list of disabled workbbooks and your Personal workbbok should be there.
Simply seletct it and the click on the ENABLE button
