Lock Wizard

Just today I was adding a text box to a form and immediately after depositing the txtBox on the form this little FORM "Lock Wizard" appeared on my screen which froze my database and allowed me three options.
This is NOT part of my database. Has anyone encountered this and what are the ramifications?
