I am new to the access world but I've been asked by my employer to create an
incident log that will track client complaints and work performance issues
related to our field employees. The form should be setup by employee and
incidents organized by date. Any help would be appreciated. Just trying to
figure out the best way to set up the form so we can retrieve employee
information for evaluations.

My advice is to review all of the templates that ship with Access along with
those that MS has OnLIne to see if there is something close to what you need.

you're putting the cart before the horse. you don't start with form design,
you end with it. you start with tables/relationships design, and take the
time to get that right first, then build queries, forms and reports on top
of that foundation. for more information, see

Step back a bit. Starting your design with Forms is a bit like putting up the
walls and frame of a house first, and then deciding where to pour the
foundations! Your Tables are *absolutely fundamental* and should be the first
thing designed and implemented.
