If a [largish] organization uses a mail code to direct mail, is
it "in the spirit" to use 'post-office-box' with the work address
to mark that up, or use the less semantic 'extended-address'?

Use 'extended-address'. A Post Office Box is
a mailbox inside a postal outlet. If you use
'post-office-box' for something else then this
can only cause confusion.

Also, if you are using HTML 5 then you might
want to consider using Microdata instead of
Microformats.
