Position Qualifications

Blue Water Community Action (BWCA) is accepting Requests for Proposals (RFP) from qualified Weatherization Energy Auditors/Inspectors to bid on Weatherization projects.  Qualified Weatherization Energy Auditors/Inspectors must have experience and equipment to complete the following:  Blower Door Testing, Combustion Appliance Inspection/Testing, Health and Safety Inspection/Testing, the National Energy Audit Tool (NEAT) WA8 and the IWC as well as any other required equipment to complete a full Energy Audit Inspection.  Approved Contractors will be placed on the Weatherization Roster as eligible to complete Weatherization jobs.  The pricing submitted with the RFP will be the pricing used with the National Energy Audit Tool to determine job awards.

POSITION SUMMARY:  Assists individuals with reaching their desired level of independence by supporting them is setting and achieving goals, sharing experiences, and providing educational and supportive services.  Contributes to the organizations’ mission of advocating for the needs of persons with disabilities and collaborating with organizations that provide services for individuals and families who have disabilities to remove system barriers and improve independence.

 

The Disability Guide will utilize the evidence-based practice of Motivational Interview approach along with the IL philosophy to maximize the independence of people with disabilities by addressing issues related to accessing systems.

 

ESSENTIAL FUNCTIONS:

·         Develop and maintain working relationships with contracting/partner community agencies and collaborating agencies through consultation with local entities such as Department of Human Services.

·         Meet with customers and service coordinators of referring community agencies.

·         Provide consultation with people with disabilities to identify barriers to education, employment, and community living through the use of assessments and motivational interviewing skills.

·         Assist people with disabilities in navigating systems by serving as their support coordinator of services insuring that unintentional barriers do not impede progress.

·         Consult service providers as necessary regarding services for their customers.

·         Provide information regarding services and programs available from Disability Network/Wayne County-Detroit.

·         Provide the four (4) core services offered by the DN/WC.

·         Maintain brochure displays at collaborating community agencies.

·         Open and maintain Consumer Service Records per DN/WC policies.

·         Assist customers with completing an Independent Living Plan if they choose to develop an Independent Living Plan.

·         Maintain knowledge of customer needs and community resources which may assist consumers in achieving their stated goals.

POSITION SUMMARY:  The Nursing Facility Transition Specialist provides direct service to consumers residing in nursing facilities who may desire to live in the community.  The Nursing Facility Transition Specialist assesses the needs of consumers, creates person-centered plans, assist consumers in their move to the community, follow-up services and outreach to Nursing Homes in Western Wayne County.  The Nursing Facility Transition Specialist also serves as an advocate at both the local and state level, and provides information and referral services in their area of service as requested.    

 

ESSENTIAL FUNCTIONS:

  • Perform outreach activities at nursing homes and community organizations to provide information to people who may want to move from a nursing home to a community-based living situation.
  • Coordinate transition activities with partner organizations as appropriate.
  • Assess consumer needs to determine supports what will be necessary for a successful transition.
  • Create a consumer directed person-centered plans with transition candidates.
  • Provide information on housing and community-based resources in consultation with DN/WC Housing Specialist.
  • Assist consumers in their move to the community and provide follow-up services.
  • Collaborate with the Senior Alliance and Detroit Area Agency on Aging to best meet the needs of consumers who desire to transition to the community.
  • Participate in Disability Network/Michigan Long-Term Care meetings and other related meetings and trainings as requested.
  • Promote self-advocacy in consumers.
  • Assist with community presentations and workshops as needed.
  • Participate in collaborative community projects on disability issues/needs.
  • Complete all applicable reports and paperwork, and prepare own correspondence, mailings, faxes, etc. as required.

Essential Functions:

  • Conduct initial phone intake and complete ServicePoint/HMIS data entry
  • Meet with case managers and potential program participants to interview and complete the intake application
  • Ensure all documentation required for intake application is collected by deadline
  • Collaborate with the housing specialist(s) from partner mental health provider agencies, as needed
  • Cross-train with Leasing Specialist role to assist with leasing-related tasks, as needed
  • Create program participant files
  • Document all case updates (Case Notes) in ServicePoint within 24 hours
  • Schedule and co-host CHN’s Renter Readiness Workshops; complete related HMIS data entry
  • Represent CHN at events in the community, as needed
  • Attend 2-3 monthly meetings on behalf of CHN
  • Assist with monthly HMIS “data cleaning” tasks, as needed

 

Qualifications/Requirements:

·         Proficient computer skills and the ability to effectively use Microsoft Office.

·         Must be organized, attentive to details and be able to multi-task in a fast-paced environment.

·         Must have reliable transportation as personal vehicles are used for job-related travel.  

·         Must be willing to conduct intakes in community settings as well as office-based environments.

LISC AmeriCorps Member Serving at Vanguard Community Development Corporation

Part-Time (900 hour in 10 months)

Ideal Start Date:  November 1, 2015

 

Local Initiatives Support Corporation and Vanguard Community Development Corporation are seeking a part-time AmeriCorps member to serve as a Community Engagement Coordinator.  The ideal candidate will begin service on November 1, 2015 and serve a minimum of 900 hours though the end of his/her term on August 31, 2016.

 

The member selected to serve in this capacity will be responsible for: maintaining contact lists, publicizing project information, and reaching out to individual residents and community organizations to increase community involvement in the decision-making for these projects. The Community Engagement Coordinator will also participate in the development of the community engagement strategy, planning of community engagement activities, and preparation and facilitation of community meetings. The Coordinator will have the opportunity to meet and learn from other community engagement professionals across the city and further develop skills and techniques to most effectively engage community members around projects identified as priorities within the BSC Quality of Life Plan.

 

Specific duties include:

  • Maintain community contact databases (using MS Access);
  • Create publicity for community meetings (such as email alerts, Facebook posts, flyers);
  • Build relationships with residents and community organizations and encourage participation in
  • project planning and implementation processes;
  • Provide input on community engagement strategy and draft agendas and activities for community engagement meetings and other outreach mechanisms;
  • Participate in the facilitation of community meetings, document meeting processes, and summarize outcomes.

 

Members perform day to day service at Vanguard Community Development Corporation but are expected to attend and participate in all LISC AmeriCorps sponsored activities including but not limited to:

  • Attending a national leadership conference tentatively scheduled for March 2016;
  • Attending all locally sponsored monthly meetings;
  • Participating in nationally sponsored webinars;
  • Actively participating in at least two locally identified and team coordinated service projects (one for Dr. Martin Luther King, Jr. Day of Service and one for National AmeriCorps Week); and,
  • Engaging in any other LISC events as determined by the local LISC office.

 

This is an AmeriCorps position.  As such neither LISC nor Vanguard Community Development Corporation will allow the member to engage in activities that are considered prohibited under the terms of the grant while serving as a LISC AmeriCorps member.

Program Eligibility Requirements

To be eligible to participate as a LISC AmeriCorps member the candidate must meet all eligibility requirements to serve as a National Service participant including but not limited to:

 

  • Possess a high school degree, GED certificate or agree to achieve GED during the term of service
  • Be at least 17 years of age (note there is no upper age limit)
  • Have proof of status as a US citizen or possess permanent resident status and be able to provide documentation as determined by CNCS (is different than documentation for employment)
  • Be eligible to earn a full-time education award
  • Be eligible to perform a term of national service
  • Be available to serve for a full 10 month period of time and within that time period complete a minimum of 900 hours of service
  • Be available for service from November 1, 2015 through August 31, 2016

 

Criminal Record Searches

If a candidate has a criminal record, it does not necessarily make a candidate ineligible for service. Only candidates who are subject to registry on the National Sex Offender Public Website or have been convicted of First Degree Murder may not perform service.  All candidates offered the position must consent to a search of the National Sex Offender Public Website and a State Level Criminal History Search. Members whose service will have them providing direct service to a vulnerable population will have to consent to an FBI level search. Vulnerable populations are defined as: (1) “children” defined as individuals aged 17 and under consistent with the PROTECT Act; (2) “elderly” defined as individuals sixty years of age (the lowest age commonly used by Congress to define elderly persons); or (3) “individuals with disabilities” as defined in the Rehabilitation Act in 29 USC 705(20)(B) and covers any individual who has a physical or mental impairment which substantially limits one or more major life activities or is regarded as having such an impairment. Vanguard Community Development Corporation attests that this AmeriCorps position will not have the candidate performing direct service to a vulnerable population.

 

Program Benefits

Upon successful completion of the term of service, the member will be eligible for a $2,865 education award to pay off existing, eligible student loans or return to school. The position pays a total stipend of $8,000.  The stipend is paid in 20 equal checks twice a month.  Direct deposit is available and highly encouraged.

The member selected to serve in this capacity will assist senior staff to: 1) assess implementation of community priorities previously identified by the community, 2) update the Quality of Life Plan, 3) work with community partners to identify resources and achievable work plans needed for implementation of community goals, 4) coordinate discrete activities included in the Quality of Life Plan such as community beautification projects, 5) continuously update implementation progress of identified community priority projects; and 6) coordinate the creation of a North End Master Plan and neighborhood revitalization strategy.

 

The Community Planning Associate will participate in community and stakeholder meetings and will have the opportunity to meet and learn from other community planning professionals across the city. The Community Planning Associate will have the opportunity to further develop skills and techniques to most effectively lead community stakeholders to identify, prioritize, articulate, and implement neighborhood revitalization projects.

 

Specific duties include:

  • Meet with community partners and assess status and implementation needs of previously-identified community priority projects;
  • Work with Community Engagement Coordinator to set the agenda and activities for community planning workshops and document outcomes of meetings;

  • Provide relevant policy research and summaries to support identified community priorities;

The member selected to serve in this capacity will be responsible for: maintaining contact lists, publicizing project information, and reaching out to individual residents and community organizations to increase community involvement in the decision-making for these projects. ·          Build or obtain internal protocols and systems necessary to manage workflow, deadlines, and compliance obligations, including asset management, vendor qualification, contract letting, and documenting compliance with all applicable federal, state, and local laws, regulations, and standards, including Fair Housing, and the requirements of any subsidized financing programs utilized.

·          Identify new partners and creative ways for the organization to increase its impact while maintaining a focus on the mission.

·          Represent the organization at speaking engagements, public hearings, and other public forums at the discretion of the Vice President of Real Estate and the CEO.

This position also includes activities like education, follow-up support, and coordination of our Project Resource Connect Event held in January. 

 

Member Responsibilities:

  • Assist with intakes to assess the housing needs of that client
  • Provide information, referrals, and assist with paperwork to individuals that are homeless who hold a Housing Choice Voucher to ensure their housing stability

PROGRAM SUPERVISOR - Supervise staff in accordance with their job duties. Train and orient new staff. Provide crisis intervention as needed. Act as back up where possible to staff in the department.  REQUIREMENTS: MA/MSW. Licensed as LMSW; or as LP; eligible for credentialing.  Relevant experience in supervision and management of professional staff. Problem solving and communication skills. CLINICIAN - This is a clinical position which involves the assessment and provision of therapy to the developmentally disabled; implementation of treatment goals and objectives; case coordination; provide linkages with other service providers; input into the development of treatment plans and on-going monitoring of consumer's progress.  REQUIREMENTS: MA/MS/MSW + one year clinical experience. Licensed Master Social Worker, Licensed Professional Counselor or Licensed Psychologist (LLMSW, LMSW, LPC, TLLP or LP). Eligible for appropriate credentialing. Experience with the Developmentally Disabled population and basic computer skills.  Valid Michigan driver’s license and proof of auto insurance.

 

LEAD CLINICAL COMMUNITY HEALTH WORKER - The position is responsible for providing outreach behavioral health case management services to consumers of Total Health Care (THC).  In addition they will provide leadership and supervision to others in the THC program.  The Clinical Community Health Worker (Lead) will assess the social and community needs of the consumer and develop an appropriate and fully integrated plan of care.  REQUIREMENTS: Graduate degree in an appropriate human service field (Social Work, Psychology, Counseling) and a minimum of two years mental health experience.  Fully Licensed LMSW, LPC, or LP required. CAADC substance abuse certification preferred.  Experience in community setting preferred. Ability to effectively provide care to socially and behaviorally complex consumers in a variety of non-traditional settings.  Experience in serving people in poor, urban environments.  Ability to work well with people of various ages, backgrounds, ethnicities and life experiences.  Ability to work collaboratively in a team and manage multiple priories, utilize effective time management skills, and exercise sound administrative and clinical judgment.  Requires the ability to travel to multiple office locations.  Experience with the mental health system and a history of services through the CMH system Preferred.  Writing skills and the ability to handle multiple tasks. Ability to effectively work with and maintain patience with the population.  Basic computer skills are required.  Experience in guiding other team members is preferred.  Valid Michigan driver’s license and proof of auto insurance.

 

SUPPORTS COORDINATOR - This is an ancillary supports coordination position involving regular face to face contact with individuals experiencing developmental disabilities. This position requires the coordination of community services rendered to clients, provision of concrete services, and advocacy tasks such as assisting in making appointments and securing financial benefits. This position requires a significant amount of paperwork.  REQUIREMENTS: LBSW + 2 years of experience in mental health field or similar degree in human services with appropriate supervision. LBSW or LMSW. Writing skills and the ability to handle multiple tasks. Ability to effectively work with and maintain patience with the population. Basic computer skills are required. Valid Michigan driver’s license and proof of auto insurance.
