Survey for my Masters Thesis

I'm currently in my last semester of my Masters studying Clinical Nutrition and am interested in studying eating behaviors of Ashkenazi Jewish women with Polycystic Ovarian Syndrome (PCOS) compared to their peers without PCOS.
Unfortunately there was an error in my original survey that had 300 respondents and I have to redo again with only a few days to gather new data. If any of you could do take the time to fill it out, I would SO SO appreciate it!
The survey is for the SOLE purpose of my class assignment and results will not be published. The survey should only take between 3-5 minutes (max, should even be less), and responses are completely anonymous.
