Release Form?

Hi Everyone,

How are you?

Does anyone already have a permission slip/release form that parents/
guardians sign acknowledging/authorizing their child's use of
technology in the classroom (ie: being videotaped/photographed,
uploading their videos to youtube, etc.)? If so, would you mind
sharing?

If you're heading back to school on Monday, good luck! If you're
still on vacation, enjoy your last few days!
