You upload your input sheet in test resources tab. In your qtp script you give the path of your input sheet in test resources.

Datatable.value(...............)

you can use Test Parameter also i.e TestArgs("Value1")

At the time of run pls put a break point and see if the sheet is getting imported into datatable and if yes then what is the name showing up.
