According to the description on the back, it appear to have been made by Fazbear Entertainment as a Logbook for Security guards to fill out in their spare time.

But so, Survival Logbook, from the point of view of someone who views it as 100% canon, is a logbook released during FNAF1 and then recieved as a handme down from there and at one point was issued to Mike.

Logbook still shows that Mike is the Fnaf 4 gameplay protagonist. I know why he has those nightmares. He was punished by his father in those rooms that William was watching and where animatronics where marked on the map.

Well, can I say one last thing? If the Survival Logbook doesn't fit with the game timeline then we can't have any clue if it's Mike. It's like Custom Night in Sister Location or FNAF World. It does have important lore bits but some of it is just for fun. The convos between the two dead kids seem legit.
